Each entry stores who made the change, the exact time it happened, and the full before-image of the record — the complete version as it stood just before the change.
No. It's enforced at the database, not the app, so it isn't a preference anyone can disable. That's the point — an audit trail you could switch off wouldn't be one your CA could rely on.
The log captures changes and deletions — the moments where a record's history matters most. The record itself, plus the log of every later change to it, together tell the full story.
It's an append-only edit log enforced by the database: entries can't be altered or removed from the app, and a correction is always a new row. It records who changed what and when — it isn't a cryptographic signature, and we don't claim to be tamper-proof in that sense.
